Abstract

Objective: To analyze the conceptual foundations, practical applications, and contemporary challenges of Evidence-Based Medicine (EBM), focusing on the tension between time, productivity, and the quality of clinical decision-making. Literature review: The literature reveals that workload, time scarcity, and the hyper-protocolization of clinical practice undermine professional autonomy and critical reflection, transforming efficiency into a value detached from prudence. Conversely, strategies such as shared decision-making (SDM), decision aids, and the ethical use of Artificial Intelligence (AI) and Clinical Decision Support Systems (AI-CDSS) have emerged as promising means of integrating scientific evidence, patient values, and contextual factors. Strengthening EBM training, developing institutional support policies, and promoting professional well-being are key components of a balanced and sustainable medical practice. Final considerations: Sustaining the triad of time, productivity, and evidence in modern medicine remains a complex yet achievable goal. Although current structures often hinder the realization of evidence-based ideals, progress is possible through coordinated strategies that integrate innovation, organizational reform, and professional well-being. The challenge is not to abandon the triad, but to rebalance it!
