Abstract

Objective: To analyze the spatial distribution and morbidity and mortality indicators of hospitalizations for diarrhea and gastroenteritis of presumed infectious origin (ICD-A09) in Brazil, focusing on the state of Maranhão, identifying geographic areas of higher risk. Methods: Ecological epidemiological study with analytical-descriptive approach, using DATASUS data from 2015 to 2024. Multilevel analysis: national (Federal Units), state (health regions), and local-regional (municipalities of Maranhão). Results: Heterogeneous distribution of hospitalizations across the national territory, with Maranhão presenting the highest prevalence (20,995.7 per million inhabitants). The health region of São João dos Patos recorded the highest hospitalization prevalence (1,182.9 per 10,000 inhabitants), Codó showed high lethality (1.78%), and São Luís demonstrated low prevalence with high lethality. Municipalities with extremely high lethality rates were identified, evidencing inequalities in hospital care quality. Conclusion: The study demonstrates profound inequalities in prevention and quality of health services, highlighting the urgent need for territorialized public policies and strengthening of healthcare networks targeted to the specific epidemiological profiles of each region for effective disease management.
