Abstract

Objective: Understand through the scientific literature the challenges for the implementation of the Full Outline of UnResponsiveness Scale in the assessment of the level of consciousness of adults and elderly in an in-hospital environment. Methods: Integrative review carried out in July 2022, in the sources: US National Library of Medicine National Institutes Database Search of Health, SciVerse Scopus, Embase, Cumulative Index to Nursing and Allied Health Literature, Web of Science and Latin American and Caribbean Literature in Health Sciences. The Rayyan program was used to select the studies and the Content Analysis. Results: 937 studies were identified, of which eight made up the final sample. Six challenges were identified, namely: difficulty in establishing procedures that reduce mortality rates, in adhering to the Full Outline of UnResponsiveness Scale, in establishing an effective assessment of the pupillary response to light in patients with ocular pathologies, in reliably evaluating restless patients in need of sedation, in carrying out an effective and reliable assessment of the pupil diameter and breathing pattern of patients affected by drug intoxications, as well as the cough reflexes of elderly patients. Final considerations: The challenges demonstrate that the Full Outline of UnResponsiveness Scale presents some weaknesses that must be overcome from the development of new studies that are able to propose behaviors that minimize the challenges for the implementation of this scale.
