Thrombolysis of thrombus inside the right atrium in transit to the right ventricle associated with pulmonary thromboembolism in an unstable patient

Abstract
Objective: To report the case of a patient with deep vein thrombosis and pulmonary thromboembolism who, after presenting syncope and clinical instability, was successfully treated by thrombolysis with alteplase, resulting in significant improvement within 72 hours. Case details: Male patient, 32 years old, obese, suffered trauma to the right lower limb in a car accident, developing DVT and was treated with rivaroxaban for 6 months. After a month, with the anticoagulant being discontinued, the patient had an episode of syncope and was taken to the health service in clear instability. The echocardiogram showed a thrombus in transit in the right atrium, right ventricular dysfunction and PTE. After thrombolysis with alteplase, there was satisfactory clinical and hemodynamic improvement. The post-thrombolysis echocardiogram highlighted the absence of thrombi and improvement in cardiac function. The patient was then discharged after a week and remained under outpatient follow-up. Final considerations: The early discovery of PTE with thrombus in transit is essential for adequate treatment, and chemical thrombolysis resulted in significant effectiveness, with adequate recovery and improvement in clinical parameters.
