Abstract

Objective: To report the case of a patient diagnosed with acute interstitial nephritis, secondary to the use of lithium in the treatment of Bipolar Disorder (TB), scientifically reviewing the correlation between the drug and nephropathies. Case detail: Female patient, 24 years old, attended in private practice reporting suicidal ideas, with a diagnosis of severe dysphoric syndrome. Sessions of cognitive-behavioral psychotherapy were initiated, as well as its drug treatment, followed by several crises and unsuccessful attempts to introduce lithium as a viable option. However, after a decade of treatment, the patient agreed to start its use. Eight years later, acute interstitial nephritis was diagnosed, secondary to the use of lithium, replacing it with carbamazepine. The patient continued her treatment for another four years, until she was referred to another professional in a different location. Final considerations: TB is a complex psychiatric disorder, with drastic mood and behavioral changes, with lithium being its first-line treatment. However, this medication is responsible for developing progressive kidney disease in a significant percentage of patients, and some of them develop severe conditions, such as interstitial nephritis.
