Abstract

Objective: To analyze advances in volume resuscitation strategies for hemorrhagic trauma, emphasizing current evidence on hemostatic resuscitation and its impact on clinical outcomes. Literature review: Hemorrhage is the leading preventable cause of death in trauma, making the early management of hemorrhagic shock essential for patient survival. Over the past decades, a paradigm shift has occurred, replacing liberal crystalloid administration with damage control resuscitation strategies, including permissive hypotension, balanced blood component therapy, early administration of tranexamic acid, and individualized coagulation monitoring. Current evidence demonstrates that these approaches reduce complications such as hemodilution, acidosis, coagulopathy, and fluid overload, while improving hemorrhage control and preserving tissue perfusion. Final considerations: Advances in volume resuscitation have transformed the management of hemorrhagic trauma by prioritizing evidence-based strategies that optimize hemostasis and reduce mortality. The implementation of updated and individualized protocols is essential to improve clinical outcomes in trauma patients.
