Abstract
Objective: To report the case of a pregnant patient with an initial picture of rhinosinusitis without adequate resolution, evolving to subdural empyema. Case details: Pregnant woman, 23 years old, without comorbidities, G5 P4 A0, did not undergo prenatal care and was a user of illicit substances. An emergency hematoma was drained, showing voluminous subdural empyema with removal of all exudation with return of the brain structures to their anatomical position, probably focused on bacterial sinusitis. The patient evolved with clinical improvement and was discharged from the hospital. With GA 33s3d, the patient was admitted to the hospital with preterm labor in the active phase, evolving to vaginal delivery, birth of a single newborn, alive, male, APGAR 8/9. The newborn was referred to the Neonatal ICU due to prematurity and the puerperal patient remained stable and uneventful. Final considerations: In view of the results presented by this study, it is observed that although most cases of sinusitis have a mild and uncomplicated natural history, it is rare for patients with an initial picture of rhinosinusitis without adequate resolution evolving to subdural empyema.
