Abstract
Objective: Review the literature on the endodontic failure with postoperative apical disease and the presence of fractured instrument and filling material in contact with the apical region, which respond positively to root canal retreatment, without the need for surgical intervention. Bibliographic review: The reviewed literature demonstrated that endodontic failure is observed when there is development, persistence or worsening of signs and/or symptoms of endodontic infection after initial root canal treatment. The treatment modalities for postoperative periapical pathologies are based on orthograde retreatment and periradicular surgery. The presence of a fractured instrument in contact with the periradicular tissues, as well as the extravasation of the filling material, are operative accidents that can happen during the instrumentation and filling of root canals, respectively, which compromise the success when associated with an infection. Final considerations: Thus, when detected in the diagnosis of postoperative disease, they are usually treated surgically, aiming at removing the material present in the apical area.
