Resumo
Objetivo: Descrever um caso clínico de osteonecrose bilateral de maxila (Estágio 3) relacionada ao uso de pamidronato dissódico (Aredia®) em um paciente oncológico e estabelecer uma breve revisão da literatura. Detalhamento do caso: Trata-se de um paciente, 78 anos, em tratamento paliativo de câncer de próstata (adenocarcinoma gleason 7) e com diagnóstico de osteonecrose dos maxilares relacionado a medicamentos (ONM) em terço médio de face. Realizou-se tratamento cirúrgico agressivo combinado com terapia adjuvante medicamentosa. Paciente evoluiu bem com resolução completa dos sintomas, ausência de recidiva e melhora da qualidade de vida. Considerações finais: A fisiopatologia da ONM não é totalmente compreendida e contém muitos fatores de risco, portanto, protocolos definitivos de tratamento ainda não foram estabelecidos. Importante observar que a remoção do osso necrótico é importante no tratamento de ONM, sendo as ressecções cirúrgicas procedimentos invasivos e necessários em pacientes em estágio avançado, especialmente em pacientes em estágio 3. O protocolo cirúrgico apresentado neste estudo mostra resultados promissores para o manejo cirúrgico de estágios avançados em pacientes com ONM de alto risco, porém, mais estudos são necessários para um tratamento definitivo e eficaz.
