Abstract

Objective: Report the experience of nursing residents in the construction of an instrument to support the nursing consultation for the child and adolescent Health Care Unit of a University Hospital. Experience report: The construction of the instrument was divided into two stages: Planning and Construction. It has six domains: 1. Current history; 2. Family socioeconomic status; 3. Personal history; 4. Basic Human Needs; 5. Nursing Diagnosis and 6. Nursing Prescriptions, based on Wanda Horta's Theory of Basic Human Needs. The research had challenges, such as condensing in a single document the main characteristics of rare diseases treated in the unit, in addition to the scarcity of scientific research on the subject. The creation of an instrument becomes opportune, since it allows to offer support to the professional during a consultation. Final considerations: The study allowed, in addition to knowledge on the subject, to acquire skills of the use of the nursing process in the most diverse areas of nursing practice with theoretical support, and also to offer a contribution to the service in a way that collaborates in care.
