Abstract

Objective: To describe a case of inferior alveolar nerve injury after a coronectomy of a third inferior molar impacted an associated dentigerous cyst. Case details: Male, age 45, presented with a cystic lesion in element 38. X-rays showed close relation with the mandibular canal and the growth of the lesion over the years with a consequential dislocation of 38. An enucleation of the cyst was conducted with the coronectomy and the roots remain buried in the bone, however, after the operation, paresthesia was found. Therapy was immediately initiated in the form of medication and infrared low-level diode laser. After two years, part of the sensitivity returned, however, the paresthesia was still persistent on the left side of the lower lip and in the inferior teeth; the roots that were already buried did not have any sign of lesion. Final Considerations: Even with precautions, when there is a close relation between a tooth and the inferior alveolar nerve, the risk of a lesion remains. The outcome of this complication varies, with the risk of temporary damage or permanent. The return of this sensitivity is multifactorial and a definitive protocol to treat paresthesia does not exist.
