Abstract
Objective: To describe the impact of health information technology on the performance of hospital pharmacists in medication reconciliation. Bibliographic review: The increase in the number of medical and health data and information, the use of information technology, and the advent of industry 4.0 in the field of health imposed a new reality and a new level for the performance of the hospital pharmacist to carry out medication reconciliation. Final considerations: The advent of Industry 4.0 made it possible to interconnect and connect numerous public and private health institutions, increasing the amount of data and information feeding big data. This intertwining of data, and information through health information technology enables the hospital pharmacist to mine data agilely and perform hospital medication reconciliation with greater precision. Information technology has opened a new professional horizon for hospital pharmacists. In Brazil, the use of health information technology for hospital medication reconciliation is still in the implementation stage.
