Abstract
Objective: To evaluate the use of vancomycin in the treatment of infections in patients admitted to the Intensive Care Unit (ICU) and to verify the adequacy of the therapeutic range after monitoring. Methods: This is a retrospective, cross-sectional, single-center study, carried out in a highly complex hospital in the North Region, from January to June 2022. Patients under 18 years of age and using oral vancomycin and/or prophylactic. Results: A total of twenty patients were included, with a mean age of 48.8 ± 19.07 years. There was a predominance of males (55%), the main comorbidities were systemic arterial hypertension (SAH) (45%) and neoplasms. A total of 29 vancokinemias were performed, of which 58.6% obtained a subtherapeutic dose, on the other hand, in patients who died (40%), there was a preponderance of the toxic dose (50%). Microbiological tests were requested for 55% of the patients, and in the positive cultures, there was a predominance of Pseudomonas aeruginosa (33.4%). Conclusion: Vancokinemia is essential for safe pharmacotherapy and should be performed in critically ill patients, especially those admitted to the ICU, to prevent therapeutic failures. The joint action of the multidisciplinary team makes it possible to optimize antibiotic therapy and reduce the progress of bacterial resistance.
