Abstract

Objective: To discuss the surgical and conservative approaches for the treatment of lumbar disc herniation, its indications and the prospect of better results. Bibliographic review: Conservative treatments include physiotherapy, based on techniques such as manual therapy, general decoaptation and axial traction, acupuncture, pilates, lumbar extension traction and transcutaneous electrical stimulation, and present satisfactory results as the first therapeutic intervention. On the other hand, surgical treatment has microdiscectomy as the gold standard, which enhances and accelerates the remission of symptoms compared to conservative treatment, has a reduced cost and lower rates of complications and reoperations. Final considerations: When comparing both approaches, it was identified that surgery is a good option when there is a need for rapid relief and for the treatment of debilitating symptoms, but, in the long term, there is no superiority of surgery over conservative treatment, since the outcome is similar. However, patients feel more satisfied with the surgical treatment, which is superior in the short term in terms of satisfaction with the procedure and improvement in disability and quality of life.
