Abstract

Objective: Identify and quantify the consumption of antimicrobials in a highly complex public hospital in the first wave of COVID-19. Methods: Descriptive, cross-sectional, and retrospective study carried out at a high-complexity hospital. Two periods were evaluated: pre-pandemic and the first wave of the pandemic. Antimicrobial consumption was expressed in Defined Daily Dose (DDD) per 1000 patients/day. The World Health Organization ATC/DDD classification was used. Results: During the pre-pandemic period, the average monthly hospital admission rate was 283 admissions; in contrast, in the first wave of COVID-19, this average reduced to 138 admissions. In the first wave of COVID-19, the most used antimicrobial was ceftriaxone, followed by piperacillin + tazobactam, cefepime, meropenem, and vancomycin. In the Intensive Care Unit (ICU), there was a considerable increase in the consumption of fourth and third generation cephalosporins, carbapenems, and broad-spectrum penicillins. Conclusion: Our findings showed a general increase in some antimicrobials, which could aggravate antimicrobial resistance. The impact of the pandemic on this issue is still unknown, raising concerns about the consequences for future generations.
