Abstract
Objective: To analyze the available evidence regarding new perspectives and therapeutic approaches in the treatment of infectious endocarditis. Methods: This study is an integrative review that follows the PVO strategy (Population, Variables, and Outcome) to elucidate effective therapies in the treatment of infectious endocarditis. The research was conducted in the PubMed database, resulting in 18 articles. Results: Intravenous antibiotic therapy is the most effective treatment for infectious endocarditis. Additionally, therapeutic approaches involving immunomodulators have shown a significant reduction in the inflammatory responses caused by infectious endocarditis, with a decrease in plasma cytokines observed through the use of hemoadsorption. However, there was no reduction in postoperative organ dysfunction, and patients often face complications associated with immunosuppression-related injuries (IAI). In the surgical aspect, patients undergoing dialysis present additional challenges due to their immunodeficiency and heightened perioperative risks; studies have indicated that surgery for these patients can be risky. Conclusion: Early diagnosis, appropriate antibiotic selection, surgical and adjuvant therapies contribute to improved clinical outcomes. However, further research is needed regarding the effectiveness of certain therapies, such as the use of oral antibiotics, adjuvant and immunomodulatory therapies, and surgical approaches in dialysis patients.
