Abstract
Objective: To report the case of systemic lupus erythematosus patient with bacterial endocarditis secondary to an intravascular device, who underwent surgery to place a tricuspid valve prosthesis and, in less than six months, presented a new infectious condition in the valve, evolving with complications and later, death; alerting to the predictive factors of poor prognosis in these cases. Case detail: Patient with Systemic Lupus Erythematosus, using immunosuppressive medication, in addition to a history of tricuspid valve replacement due to endocarditis correlated to the use of the Portocath device. Seek medical care and further exams with detailed results of active lupus and new bacterial endocarditis in a prosthetic valve. During hospitalization, the patient presented several complications, showing no clinical improvement even with therapeutic optimization, resulting in death. Final considerations: Infective endocarditis has a high mortality rate even with early diagnosis and treatment. The presence of factors such as: comorbidities, use of immunosuppressive medication, early valve reinfection, large vegetation, growth despite antibiotic therapy and embolization, predicted a poor prognosis in this case, culminating in death as the outcome.
