Abstract

Objective: To explore the current literature on the bioethical aspects associated with nutrition and artificial hydration in patients under palliative care. Literature review: The main objective of palliative care is to improve the quality of life of patients facing serious and incurable illnesses, preventing and alleviating suffering in various spheres, such as physiological, psychosocial and spiritual, making the management of their nutrition and hydration a complex and delicate issue. Patients in palliative care may undergo changes in the route of feeding, from oral to artificial, so switching from oral to artificial feeding has a significant impact on the patient's life and habits, as well as being considered an invasive method that carries a significant risk, and its use should be cautious. Thus, nutritional support and artificial hydration in palliative care require constant discussions on ethics and bioethics, requiring a shared, complex and interdisciplinary reflection, as they deal directly with pain, loss and suffering in the face of the imminent death of patients with no therapeutic possibilities for a cure. Final considerations: The paradigms between initiating or suspending hydration and nutrition in patients at the end of life are challenges that involve ethical issues, so knowing the principles that govern bioethics is opportune to provide individualized care, and above all respect for the patient's autonomy, which are pillars that can help us make decisions in this dying process.
