Abstract
Objective: Provide considerations on the potentialities and difficulties of using oral anticoagulants in the prophylaxis and treatment of thromboembolic events. Experience report: The study discusses experiences during the Medical Residence process in a tertiary anticoagulation outpatient clinic. In the months of July and August 2023, adults and elderly people who have in common the need for prolonged use of oral anticoagulants received support. During this experience, holistic and individualized care, health education and inclusion of patients in their health-disease process were practiced. Throughout the care, some patients were adherents of first-line therapy and were discharged to Primary Health Care. With this study, it was possible to propose a reevaluation so that Direct Oral Anticoagulants form part of the list of medications made available by the public system. Final considerations: Holistic management and inclusion of the patient in their health-disease process are fundamental to good clinical practice, as these actions strengthen the doctor-patient relationship and increase adherence to the proposed treatment. This study seeks to stimulate the production of knowledge and provides dialogue with current literature to compare well-established concepts with real clinical practice.
