Abstract
Objective: To evaluate the acute responses to exercise with a cycle ergometer in the postoperative period of elderly and long-lived elderly patients undergoing transcatheter aortic valve implantation (TAVI), including the safety and feasibility of physical exercise. Methods: Non-randomized clinical trial with 39 patients. Blood pressure (BP), heart rate (HR), perceived exertion and functional parameters (frailty: 5-meter walk test – T5M and functional capacity: 6-minute walk test – 6MWT) were collected in pre-TAVI moments, post-TAVI (first postoperative day) and at hospital discharge. Post-TAVI, a physical exercise protocol with an upper limb cycle ergometer was carried out in a single session with two sets of five minutes. Results: After exercise, there was a change (p=0.001) in systolic BP (127.28±17.52 vs 138.36±19.20mmHg), HR (72.36±12.83 vs 81.13±17, 92), Borg dyspnea (0.62±1.17 vs 2.49±1.52) and Borg fatigue upper limbs (0.50±0.80 vs 3.08±1.49). The hemodynamic behavior during exercise was similar between the elderly and the oldest old. Frailty and functional capacity worsened between pre- and post-TAVI, but improved at hospital discharge. Conclusion: Cycle ergometer exercise was safe, viable and proved beneficial in improving frailty and functional capacity for hospital discharge. There were no records of adverse events.
