Abstract
Objective: To describe the importance of patient safety culture and adverse event reporting practice for incident mitigation. Method: This is an integrative review with content analysis of the articles selected after searching the Virtual Health Library (VHL) and National Library of Medicine/Medical Literature Analysis and Retrieval System Online (PUBMED/MEDLINE) database searching, using the following descriptors: patient safety, notification and culture. Results: Thirteen articles were defined through the analysis and inclusion and exclusion criteria for the final elaboration of the study. Discussion: From the content analysis five categories were highlighted: Organizational processes for patient safety culture; Professional's perception of patient safety; Patient safety culture; Factors of occurrence of adverse events and Adverse Event Reporting. Final considerations: Further studies on patient safety are needed, as there is still a prevalence of punitive adverse event reporting culture, which influences underreporting by professionals who witness events. Thus, management, awareness, continuing education, monitoring, communication and especially the involvement of leaders, supervisors and managers are required.
