Abstract

Objective: To describe the role of professionals from the Family Health Strategy in health education and improvements in the lifestyle of diabetics and hypertensive patients. Experience report: The escape from the disease-centered health model found in the territory of Primary Health Care (PHC) a space conducive to the development of actions aimed at the user. The Bem Viver entitled group met monthly, where different themes were worked on and developed in six meetings to learn about lifestyle perceptions based on educational health strategies. Health promotion becomes emancipatory practices as the individual becomes the protagonist of the reality in which they are inserted. Educational actions enhance the subject as a manager of habits and life care. Final considerations: It was observed that the success of health education actions depends on the joint work of all team members and that the impact of this approach on adopting a better lifestyle is the empowerment of patients to participate in the process of care. These activities also increase the patient's bond with health professionals, developing a feeling of belonging and security with the behaviors.
