Abstract
Objective: To analyze fluid therapy in trauma victims by using current protocols and recent bibliographic references. Bibliographic Review: Fluid Therapy aims to correct blood volume and hydroelectrolytic disorders which are considered basic elements for preventing the lethal triad (metabolic acidosis, hypothermia and coagulopathy). This approach in trauma has undergone drastic changes in the last years, including the concept of permissive volume replacement in which aggressive and continuous fluid infusion is no longer applied in a standard way, thus avoiding reactivating bleeding by competing with the physiological process of interrupting bleeding. Final considerations: There have been constant updates to the consensuses concerning the management of fluid therapy. This being the case, the recycling of medical approach for fluid therapy in polytraumatized patients must be dynamic and permanent. Within the evidence are the limitation of permissive volume replacement, the incentive to use Focused Assessment with Sonography for Trauma (FAST), the accurate prescription of tranexamic acid and the infusion of blood components.
