Abstract
Objective: To describe the conservative treatment technique in teeth with large periapical lesions, whose conduct was the performance of endodontic retreatment associated with apicoectomy surgery. Case report: A 31-year-old female patient with pheoderma presented a periapical lesion of approximately 15mm associated with the upper right central incisor. The treatment of the case was carried out in two stages: the first aimed at relieving the patient's pain (lesion decompression), followed by removal of the filling material and surgical re-instrumentation, via canal and the second, consisting of apicoectomy surgery followed by padding with MTA. Final considerations: It was possible to observe that paraendodontic surgery is an excellent ally as a conservative treatment in situations where other options have already been exhausted and that the interaction of several factors, such as diagnosis, surgical technique, and endodontic retreatment contribute to the success of the case.
