Abstract
Objective: To review the literature and gather solid data on mechanical ventilation (MV) strategies based on low tidal volume to support safe decision-making and ensure best bedside practices in intensive care units (ICUs). Literature Review: Acute Respiratory Distress Syndrome (ARDS) is characterized by diffuse alveolar damage and the development of non-cardiogenic pulmonary edema due to increased alveolar-capillary membrane permeability. Although pulmonary edema is typical in the early stages, the injury progresses through three pathological phases: exudative, proliferative, and fibrotic. The characteristic radiological finding is diffuse, bilateral alveolar infiltrates. Few therapies have shown true efficacy, making early identification, accurate diagnosis, and treatment of underlying triggers critically. The use of a tidal volume ≤ 6 mL/kg of ideal body weight is one of the most common strategies. Final Considerations: Planning MV with low tidal volume strategies can improve outcomes. However, further research is needed to identify additional variables that may influence clinical practice.
