Abstract

Objective: To report a case of nephrotic syndrome triggered after anti-COVID vaccination in an elderly patient living in the interior of the Brazilian Amazon. Case detail: A 72-year-old male patient from Santarém, a city in the interior of the Amazon, developed nephrotic syndrome three days after receiving the anti-COVID vaccine. The diagnosis was consolidated only two months after the start of the clinical condition, after investigating the probable causes of generalized edema. Based on the kidney biopsy, light microscopy showed no alterations compatible with the patient's signs and symptoms, so an electron microscopy evaluation was ordered, which revealed podocyte degeneration, retraction of the pedicels and absence of electrodense deposits, findings that need to be associated with the patient's clinical presentation in order to conclude the diagnosis.  The patient was treated with an immunosuppressive dose of prednisone and progressed with remission of proteinuria and resolution of the clinical condition. Final considerations: The importance of anti-COVID vaccination is unquestionable, since despite millions of doses being administered, the side effects in comparison are minimal. However, it is important to highlight the possible relationship between the immunizer and nephrotic syndrome in order to aid clinical reasoning and speed up patient treatment.
