Abstract

Objective: To report the case of a patient who had compression of the ulnar nerve in Guyon's canal due to the presence of a traumatic pseudotumor lesion. Case details: A 44-year-old male bricklayer came to the clinic complaining of pain in his right hand, tingling and impairment of his ability to work as a result of trauma to the hypothenar region, followed by paresthesia in the 4th and 5th right fingers and a strength deficit, which had occurred around 100 days previously. The patient was referred for a nuclear magnetic resonance scan, which showed mild tenosynovitis of the flexors of the 5th finger and a finding suggestive of a traumatic aneurysm and the presence of a pseudotumoral cyst in the superficial palmar arch at the level of the hypothenar region. The patient underwent neurolysis surgery, through an incision in the anterodistal region of the forearm and right hypothenar region, with resection of the pseudotumor together with the ulnar artery. After 6 months of surgery, he reported pain relief and improvement in paresthesia. Final considerations: In most cases, compression of the ulnar nerve is the result of microtraumas, but the case presented is considered rare, since the pseudotumor lesion that led to Guyon's canal neuropathy was the result of a specific trauma.
