Abstract

Objective: To report the case of a patient diagnosed with grade I follicular lymphoma, which progressed to diffuse large B-cell lymphoma (DLBCL) with involvement of the head of the pancreas. Case details: A 57-year-old male patient with a history of grade I follicular lymphoma, treated with chemotherapy (RCVP protocol) and radiotherapy. In February 2024, he was admitted with jaundice and abdominal pain, and was diagnosed with DLBCL, involving the head of the pancreas, hepatic and retroperitoneal lymph nodes. Radiological exams revealed a pancreatic lesion with cystic degeneration, compression of the common bile duct, and dilation of the bile ducts. Conclusion: This case illustrates the rarity of primary pancreatic non-Hodgkin lymphoma and its progression from grade I follicular lymphoma to diffuse large B-cell lymphoma with pancreatic involvement. Early detection through imaging exams is crucial for proper diagnosis and management. The combination of chemotherapy and radiotherapy was initially effective, but progression to pancreatic complications highlights the aggressive nature of the disease. Continuous surveillance is necessary, considering the possibility of recurrences, and management requires a multidisciplinary and personalized approach to optimize clinical outcomes.
