Abstract
Objective: To report a case of acute myocarditis that overlapped Chagas Disease and Cytomegalovirus. Case details: A 42-year-old male patient presented with persistent fever for 28 days, chest pain, headache, and palpitations, denying any comorbidities. Initial tests showed repolarization abnormalities on the electrocardiogram and elevated troponin (0.39), along with leukocytosis. During hospitalization, imaging studies revealed acute non-ischemic myocarditis with late enhancement, and serology indicated acute infection with Cytomegalovirus. The patient developed cardiac tamponade, necessitating pericardiocentesis due to a large pericardial effusion, and underwent a reversal of PCR. He progressed to atrial fibrillation and renal failure, requiring hemodialysis in the Intensive Care Unit. After further attempts to control the infection, he was diagnosed with Chagas Disease and started on treatment with Benznidazole. The patient showed clinical improvement and was discharged with instructions for cardiac medication use and continuation of treatment at home. Final considerations: Further robust studies are needed on protocols addressing myocarditis due to concurrent infections.
