Abstract

Objective: To report a case of a patient with an anomalous origin of the right coronary artery from the left coronary sinus and with a malignant interarterial pathway Case details: This is an observational and descriptive study, in the form of a case report, regarding the malignant course of the right coronary artery in a female patient with no history of previous comorbidities, an athlete, and symptomatic at initial evaluation. The diagnostic confirmation was achieved through coronary CT angiography. The patient underwent myocardial revascularization surgery via a mammary graft to the right coronary artery, resulting in clinical improvement of the initial precordial pain. Final considerations: It is considered that coronary CT angiography is the gold standard for evaluating coronary anomalies, which are strongly associated with sudden death. Although there is no consensus regarding the surgical approach, therapeutic decisions should be individualized based on each patient's clinical presentation, making this intervention a favorable option, especially in symptomatic patients with documented myocardial ischemia on cardiological exams.
