Abstract

Objective: To analyze the implementation of the antimicrobial stewardship program (ASP) in an emergency room. Methods: A prospective, descriptive, and quantitative study that analyzed the implementation of the ASP in an emergency room. Results: 385 patients with a mean age of 68.52 years and 54.03% male were followed. The length of hospital stay was 18.87 days and 5.27 days in the emergency room. The most common diagnoses were heart failure (28.83%) and acute myocardial infarction (18.45%). The most common infections were community-acquired pneumonia (38.18%) and nosocomial pneumonia (19.74%). Ceftriaxone (21.80%) and piperacillin + tazobactam (21.70%) were the most prescribed. The DOT per thousand patient-days for piperacillin + tazobactam ranged from 104.45 to 187.01 and for ceftriaxone from 61.57 to 129.07. The main pharmaceutical interventions were dose adjustment (30.53%) and reduction of treatment time (20.63%). Discharge was associated with older men and with longer hospital stays, while death was linked to advanced age and previous hospitalization. Conclusion: The absence of records hindered the conduction of a more detailed preliminary analysis. However, the results obtained reinforce the importance of implementing the PGA in the emergency department, as this contributed to the optimization of antimicrobial use in this scenario, as well as favoring the clinical outcomes of the monitored patients. These findings indicate a positive impact both on care practices and patient safety, highlighting the relevance of the continuity and strengthening of the program in the service.
