Abstract

Objective: The study seeks to critically analyze the scientific evidence on the neurological repercussions of heart failure (HF), focusing on structural and functional changes detected by neuroimaging, and their relationship with cognitive dysfunctions. Methods: This is an integrative review of the literature, conducted with methodological rigor. Articles published between 2019 and 2024 in scientific databases, including MEDLINE and LILACS, were selected using the DECS/MESH descriptors: "Heart Failure", "Brain" and "Neuroimaging", including their synonyms. Strict inclusion and exclusion criteria were applied, resulting in eight articles analyzed in detail. Results: The reviewed studies indicate that patients with HF have reduced cerebraperfusion, cortical atrophy, hyper intensities of white matter and changes in functional connectivity, particularly in the prefrontal cortex and in regions of the limbic system. These structural changes are associated with cognitive decline, autonomic dysfunction and increased risk of neuropsychiatric disorders. Final considerations: Neuroimaging reveals significant impacts of HF on the brain, reinforcing the need for therapeutic strategies that integrate cardiological and neurological approaches. Future studies should deepen the underlying mechanisms and investigate interventions to mitigate cognitive impairment.
