Abstract

Objective: To describe the context of vasectomies and tubal ligations in Brazilian federated units (FU) regarding socioeconomic and assistance aspects and correlate with the Municipal Human Development Index (MHDI). Methods: This is an ecological study, with data aggregated on a territorial basis. The numbers of vasectomies and tubal ligations by FU were extracted from Department of Information Technology of the Unified Health System (DATASUS), from 2010 to 2023. Then, the ratio of vasectomies to tubal ligations was calculated and compared with the MHDI of the FU. Results: The public health system (SUS) showed a high predilection for female interventions between 2010 and 2023, assuming 0.6 the ratio of vasectomies to tubal ligations in Brazil, tended to be lower in FU with lower MHDI. Spearman's correlation coefficient was 0.739, with p<0.01, statistically significant. The highest rates of vasectomies/tubal ligations were found in the states with the highest MHDI. Conclusion: Vasectomy, despite being the simplest method, which presents the lowest risk and the greatest savings for the user and the SUS, continues to be very rarely performed in regions with the lowest MHDI. Additional efforts need to be made to include men in family planning services in the poorest areas of Brazil.
