Abstract

Objective: To report the experience of a university extension project aimed at promoting the health of individuals with non-communicable chronic diseases (NCDs) through educational activities developed within a Family Health Strategy (FHS) unit in the municipality of Belém, Pará, Brazil. Experience report: The project was carried out between 2023 and 2025 by medical students within the axis of Interaction, Teaching, Service, and Community (IESCG), focusing on users with systemic arterial hypertension (SAH) and type 2 diabetes mellitus (T2DM) enrolled in the HIPERDIA program. Activities included discussion groups, educational dynamics (such as themed bingo, true or false games, and word searches), and the monitoring of vital signs both at the health unit and in the community. Students faced challenges such as low engagement from the FHS team, limited institutional guidance, and structural barriers to conducting activities outside the health unit. Nevertheless, positive outcomes were observed, including demystification of medication use, encouragement of self-care, and increased user engagement in treatment. Final considerations: The experience highlighted the transformative potential of university extension in fostering critical student development and enhancing the quality of Primary Health Care, by strengthening bonds and promoting more participatory and effective practices in the management of NCDs.
