Abstract

Objective: To evaluate the effectiveness of denosumab in the treatment of postmenopausal osteoporosis, focusing on bone mineral density and possible metabolic changes. Method: An integrative literature review was conducted using the PUBMED/MEDLINE, ScienceDirect, SciELO, and BVS/LILACS databases, including studies from the past 10 years. Data collection took place between December 2024 and January 2025, using the descriptors “Postmenopausal Osteoporosis,” “Metabolism,” and “Denosumab.” Results: Eight studies were included, totaling 33,505 women. The results indicate that denosumab significantly reduces the risk of osteoporotic fractures and increases bone mineral density, with reduced osteoclastic activity, evidenced by markers such as total alkaline phosphatase and osteocalcin. An increase in triglyceride levels was also observed between the third and sixth months of use, indicating the need for lipid monitoring. Cases of hypocalcemia were reported, particularly among patients with comorbidities or concomitant medication use. Final Considerations: Compared to bisphosphonates and placebo, denosumab showed good tolerability and a low incidence of serious adverse effects. It is concluded that denosumab is an effective and safe alternative for postmenopausal osteoporosis, although monitoring for metabolic changes is recommended.
