Abstract
Objective: To describe a clinical case of bilateral osteonecrosis of the jaw (Stage 3) related to the use of pamidronate disodium (Aredia®) in an oncology patient and to establish a brief review of the literature. Case details: This is a 78-year-old patient undergoing palliative treatment for prostate cancer (adenocarcinoma Gleason 7) and diagnosed with drug-related osteonecrosis of the jaw (ONJ) in the midface. Aggressive surgical treatment combined with adjuvant drug therapy was performed. The patient evolved well with complete resolution of symptoms, absence of recurrence and improvement in quality of life. Final considerations: The pathophysiology of ONJ is not fully understood and contains many risk factors, therefore, definitive treatment protocols have not yet been established. It is important to note that the removal of necrotic bone is important in the treatment of ONJ, with surgical resections being invasive and necessary procedures in advanced-stage patients, especially in stage 3 patients. The surgical protocol presented in this study shows promising results for the surgical management of advanced stages in high-risk ONJ patients, however, further studies are needed for definitive and effective treatment.
