Abstract
Objective: To analyze the impact of chronic endometritis (CE) on female infertility by investigating its etiologies, diagnostic methods, and therapeutic strategies, with a focus on improving reproductive outcomes. Methods: An integrative literature review was conducted using the PubMed, SciELO, and Virtual Health Library (VHL) databases, with the descriptors “chronic endometritis” AND “infertility.” A total of 16 articles (clinical trials, systematic reviews, and observational studies) published between 2020 and 2025 were included, after screening based on PICO criteria. Results: CE was found to be associated with bacterial infections and endometrial dysbiosis. Hydrosalpinx and adenomyosis were also identified as factors increasing CE prevalence. The combination of hysteroscopy and immunohistochemistry enhances diagnostic accuracy. Antibiotic therapy (with doxycycline as first-line treatment) significantly increases live birth rates. Innovative therapies such as recombinant type III collagen, prebiotics and probiotics, vaginal microbiota transplantation, platelet-rich plasma, and stem cell therapy have emerged as promising alternatives for immune modulation and tissue regeneration. Final considerations: CE is a reversible cause of infertility, and its management requires early diagnosis using combined methods, personalized antibiotic therapy based on cultures, and surgical correction of associated comorbidities. Future studies should standardize diagnostic criteria and explore regenerative therapies.
