Abstract

Objective: To estimate the Maternal Mortality Ratio (MMR) due to hypertensive complications in Maranhao. Methods: An ecological study on deaths caused by hypertensive complications during the pregnancy-puerperal period, recorded by health regions (HR) of Maranhão between 2019 and 2023. Data were extracted from the DATASUS platform, specifically from the Mortality Information System and the Live Birth Information System. Results: A total of 134 deaths due to hypertensive complications were recorded, mainly attributed to eclampsia (57.5%) and preeclampsia (29.9%). The average MMR in the state for this cause was 25.5 per 100,000 live births (SD = 5.3). The regions with the highest rates were Barra do Corda (MMR = 64.2), Bacabal (MMR = 51.2), and Caxias (MMR = 39.5). The predominant sociodemographic profile was among mixed-race women (73.1%), with 8 to 11 years of schooling (47.8%), single (46.3%), and aged 20 to 39 years (70.9%). The spatial distribution of MMR revealed heterogeneous dispersion, with higher rates concentrated in the central and eastern areas. Conclusion: Hypertensive complications are a significant cause of maternal mortality in Maranhão, with a consistent pattern of deaths that highlight regional inequalities. Greater public policy attention is needed, especially for the most affected areas, in order to implement effective interventions.
