Abstract

Objective: To identify predictors of mortality in elderly individuals with proximal femur fractures undergoing surgery within one year. Method: Study with 142 patients ≥60 years old, admitted to a referral hospital for orthopedic trauma in the main macro-region of coverage. Univariate analyses (chi-square, Cox regression) and Kaplan-Meier, Log-Rank, Breslow, and Tarone-Ware methods were used. Results: Mortality was 30.5%, prevalent in women (23.4%), with comorbidities (28.1%), trochanteric fracture (16.3%), left side affected (17.7%), and use of intramedullary nail (16.3%). There was higher mortality among patients with ASA I/II (28.4%), hemoglobin <12 g/dL (21%), no blood transfusion (23.4%), INR >1 (19%), hospital stay <7 days (22.7%), ICU admission (20.6%) and no gait training (23.4%). The Cox analysis indicated age >85 years (HR=3.31; p=0.034), ICU admission (HR=2.54; p=0.009) and lack of gait training (HR=4.12; p<0.0001) as factors significantly associated with mortality. Conclusion: Advanced age, ICU admission and lack of gait training before hospital discharge are predictors of death within one year after proximal femur fracture surgery in the elderly.
