Abstract

Objective: To quantify cases of adequate adherence and abandonment of anti-HIV therapy and identify the factors associated with this behavior in a referral hospital in Belém, Pará, between 2019 and 2023. Methods: A cross-sectional, analytical-descriptive study based on data from patients living with HIV treated at a referral hospital for infectious and parasitic diseases in northern Brazil. Results: The sample included 360 patients, predominantly male, aged between 31 and 50 years, single, with a high school diploma, diagnosed between 2016 and 2020, and residing in the Metropolitan Region of Belém. It was observed that 46.7% of participants were adherent and 53.3% abandoned clinical and drug follow-up in at least one year of the analyzed period; however, in the annual analysis, the number of adherents exceeded that of non-adherents. Adherence was associated with the presence of comorbidities, absence of opportunistic diseases, undetectable viral load, and CD4 count above 200 cells/mm³. Conclusion: HIV treatment adherence in the hospital studied was lower than the national average, highlighting specific regional challenges in the Brazilian Amazon, marked by its own social and structural contexts.
