Abstract

Objective: To report the case of a young patient with multiple symptomatic uterine leiomyomas, managed conservatively with goserelin acetate (Zoladex®). Case details: A 28-year-old nulliparous female patient presented with abnormal uterine bleeding, pelvic pain, and a diagnosis of multiple uterine myomatosis, with lesions classified between FIGO 2 and 5. At that time, pharmacological treatment with Zoladex® 10.8 mg was chosen, in two doses administered between August 2021 and March 2025. A satisfactory clinical response was observed, with a reduction in uterine volume from 553.2 cm³ to 94.3 cm³ (−82.95%) and improvement in symptoms, without the need for immediate surgery. After follow-up with new consultations, it was noticeable that the adverse effects were mild and controllable, allowing treatment to continue. Final considerations: The case shows that goserelin is an effective and safe alternative for tumor reduction and symptomatic control in patients with uterine myomatosis and gestational desire, constituting a promising conservative strategy when immediate surgery is undesirable or contraindicated.
