Abstract
Objective: To present a case report of ovarian dysgerminoma in an adolescent treated in the public healthcare system of the Federal District, to analyze the survival of patients undergoing conservative surgical treatment, with focus on current evidence. Case details: A 17-year-old female patient presented to an emergency department in the Federal District with severe pelvic pain associated with progressive abdominal enlargement. Following investigation through imaging studies (ultrasonography and pelvic magnetic resonance imaging) and tumor markers, the patient underwent exploratory laparotomy with left salpingo-oophorectomy, with intraoperative capsular rupture. Histopathological and immunohistochemical diagnosis confirmed ovarian dysgerminoma, FIGO stage IC1. The patient was referred for surveillance without adjuvant chemotherapy and is currently under follow-up with tumor marker monitoring. Final considerations: Conservative surgery with surveillance without initial adjuvant therapy constitutes the standard approach for dysgerminoma stage IA-IC adolescents, allowing high survival rates and preservation of fertility comparable to the general population. Therapeutic decision-making should be individualized and multidisciplinary, considering staging, the desire for fertility preservation, and risk factors.
