Abstract

Objective: To systematically analyze the scientific literature on surgical techniques used in the treatment of chronic scapholunate ligament injuries, summarizing outcomes related to pain, strength, range of motion, function, radiographic parameters, and complications. Methods: This systematic review was conducted according to PRISMA 2020 guidelines and registered in PROSPERO (CRD420251086731). A comprehensive search was performed in MEDLINE/PubMed, LILACS, CENTRAL, Web of Science, Embase, Scopus, and SciELO, complemented by gray literature. Studies including adults (>18 years) with chronic scapholunate ligament injuries treated surgically, with a minimum follow-up of three months and objective outcome assessment, were included. Results: Of the 1,385 records identified, 39 studies were included, comprising 1,364 patients. Most studies were case series, followed by cohort studies and one randomized controlled trial. The main surgical techniques were capsulodesis, tenodesis, combined procedures, and partial arthrodeses. Pain reduction and functional improvement were commonly reported; however, partial loss of range of motion and progressive radiographic instability were observed over time. Conclusion: No surgical technique can currently be considered the gold standard for chronic scapholunate ligament injuries, highlighting the need for randomized controlled trials.
