Abstract
Objective: To describe the perception of primary care professionals regarding assistance in the hospital-home transition of elderly people. Methods: Qualitative study based on the framework of transition theory. Nine professionals from a basic health unit in the Southern region of Brazil participated. The participants were from the family health strategy team and the multidisciplinary team. Data collection took place through interviews from March to November 2024. Results: The categories that emerged were: the context of the elderly person after discharge and the challenges for family members responsible for care; the organization of primary health care in supporting post-hospital discharge care for elderly people; the professionals' perception of the challenges and pathways for comprehensive care for elderly people after hospital discharge. Conclusion: The transition of care for dependent elderly people after hospital discharge to home occurs inefficiently in the context of primary health care. Some strategies can be implemented to minimize the discontinuity of care after hospital discharge.
