Abstract

Objective: Gain a general understanding of the diagnosis and pathogenesis of Philadelphia-negative myeloproliferative neoplasms. Literature review: Thrombosis is the most immediate threat to the health of patients. This means that patients with myeloproliferative neoplasms have an increased risk of developing thrombosis to the general popuation. Current therapeutic approaches aim to decrease the risk of thrombosis with antiplatelet agents, therapeutic phlebotomy, and cytoreductive therapies such as hydroxyurea. Given the inherent risk of thrombosis in PV and TE, regardless of the current risk stratification, the choice has been made to initiate cytoreductive therapy. The discovery of the JAK2 activating mutation (V617F), present in more than 70% of patients with MPNs, enabled the development of new biochemically selective JAK2 inhibitors. One of the JAK1/2 inhibitors is ruxolitinib which is a non-myelotoxic agent that does not kill hematopoietic stem cells. Final considerations: Despite advances, it is necessary to search for new therapeutic agents that offer good action and fewer side effects.
