Abstract

Objective: To evaluate the agreement and accuracy of tumor size measurements obtained by ultrasound and mammography when compared to the histopathological gold standard in patients undergoing breast cancer surgery. Methods: This was a field and documentary research, based on a review of medical records and ultrasound, mammography, and histopathological reports of patients undergoing breast cancer surgery at Hospital do Bem, in Patos, Paraíba, from January 2025 to December 2025. Data collection was performed through analysis of medical records archived at the hospital. A standardized instrument (Google spreadsheets) developed by the authors was used. Results: The strongest association was between ultrasound and mammography (ρ = 0.604), while the weakest was observed between mammography and histopathology (ρ = 0.439). The findings indicate a moderate association, with ultrasound estimates showing greater proximity to the histopathological standard. There was no significant difference between ultrasound and histopathology (p = 0.202). In contrast, mammography differed significantly from histopathological examination (p = 0.008), indicating a discrepancy in tumor size estimation. Conclusion: Ultrasound showed better relative performance, with greater accuracy and lower estimation error, while mammography showed greater discrepancy.
