Abstract
Objective: To report a rare case of mesenteroaxial gastric volvulus associated with intestinal malrotation in a young adult, emphasizing the diagnostic challenges, computed tomography findings, and the therapeutic approach adopted. Case details: A 26-year-old male patient presented with vomiting, abdominal distension, and cessation of fecal and flatus passage. Computed tomography and upper gastrointestinal endoscopy established the diagnosis of mesenteroaxial gastric volvulus associated with gastric ischemia. Laparoscopy revealed intestinal malrotation and splenic laceration. Following splenectomy and surgical reduction of the gastric volvulus, the patient showed clinical improvement. Final considerations: Acute gastric volvulus is a rare cause of acute abdomen in the emergency department. Due to its potentially severe complications, this condition requires rapid diagnosis and prompt intervention, as delayed recognition is associated with increased morbidity and mortality. Computed tomography plays a pivotal role in confirming the diagnosis and guiding therapeutic planning. Early decompression and, when indicated, surgical intervention are essential to reduce complications and improve patient outcomes.
