Abstract
Objective: To report a case of mandibular angle fracture occurring during the extraction of an impacted mandibular third molar and to discuss risk factors, management, and key therapeutic alternatives in light of the literature. Case details: A 20-year-old female patient underwent the extraction of an impacted tooth 38, classified as Pell and Gregory class IIB and Winter mesioangular. During luxation—performed without odontosection—an unstable, non-displaced fracture of the mandibular angle occurred, accompanied by loss of occlusion and paresthesia. Immediate maxillomandibular fixation was performed, followed one week later by open reduction and internal fixation using two 2.0 mm system plates. Postoperative progress included limited mouth opening during the first month (managed with physical therapy), absence of infection, partial improvement of paresthesia, satisfactory healing, and restoration of masticatory function. Final considerations: Immediate recognition and individualized management enabled satisfactory stabilization, bone repair, and functional recovery. Preoperative planning, assessment of risk factors, and treatment selection based on fracture stability are crucial for a favorable prognosis.
