Case details: Male patient, 47 years old, motorcycle taxi driver, suffered a collision: motorcycle with a truck artifact. The patient presented bilateral vesicular murmur during the screening and, with the complementary exams, it was seen that he had the presence of a left pleural effusion, a pericardial fluid, a hematoma in the mediastinum, enlarging the upper portion of it and atelectasis of the parenchyma. An emergency thoracotomy was performed, detecting a lesion in the right atrium, at the entrance of the superior vena cava, and, in the postoperative period, the patient was intubated and had drains bilaterally in the chest. Final considerations: Even though it has been seen for centuries, cardiac trauma continues to present itself as an important topic and should be detected as soon as possible. In the present report, we will see a case that showed itself in an atypical way, however, it was quickly identified and treated.
