Abstract

Objective: To carry out the Systematization of Nursing Assistance to a patient with sequelae of Stroke accompanied by the Best at Home Program in a city in the state of Pará. Details of the case: Patient, MFC, 69 years old, female. The daughter reports that two years ago, during lunch, the patient had an episode of peripheral paralysis on the right and lethargy. Because she considered it a momentary occurrence, she did not immediately seek medical attention. One week later, the patient experienced an episode of emesis, evacuation, diuresis and prostration. Given this, the daughter took her to the hospital, where after medical evaluation she was medicated, she was under observation for 12 hours and was discharged. At home, the patient started a gradual evolution of cognitive deficit, evidenced by aphasia, lethargy and difficulty walking. After a history of hospital admissions, the patient started a care plan proposed by the multidisciplinary team of the Best at Home Program. Final considerations: The application of the nursing process to the patient at home with sequelae resulting from stroke in this study, made it possible to demonstrate how effective this strategy is when used in the nursing care of home care services, providing holistic assistance and with scientific basis.
